Access Your Child's Health Information in MySaskHealthRecord
Parents and legal guardians can now request permanent access to the health information of their child(ren) under the age of 14 in MySaskHealthRecord. To request access, please follow the steps below.

Sign Up for MySaskHealthRecord
You must have your own MySaskHealthRecord account in order to request access to your child(ren)'s health information in MySaskHealthRecord, as your child's information will be directly tied to your account.
Complete the Request Form
Once you have successfully signed up for MySaskHealthRecord, please review and complete the
Request Form to access your child's information in MySaskHealthRecord:
Collect Supporting Documentation
You will need to submit proof of parentage/guardianship documentation with your completed request form. Please refer to Section 3 of the form, but some examples include a long-form birth certificate, adoption order, or guardianship order.
