The Chronic Disease Management – Quality Improvement Program's goal is to give health care providers tools and easy access to information to encourage the adoption of best practices to provide the very best care for their patients living with chronic conditions.
The program covers the following conditions:

Diabetes

Coronary Artery Disease (CAD)

Heart Failure (HF)
-
Chronic Obstructive Pulmonary Disease (COPD)
The program encourages and supports physicians – and other health care providers – to share and use the best medical practices.
The program is a cooperative between the Saskatchewan Medical Association (SMA), the Ministry of Health and eHealth Saskatchewan.
The Ministry of Health and SMA jointly oversee the program. The Ministry takes the lead role in physician compensation. The SMA takes the lead role managing clinical information.
Both eHealth Saskatchewan and the SMA are jointly responsible for service delivery and for protecting the privacy of program information.
