This application/registration is for the COVID-19 vaccine ONLY. For vaccine information, refer to online COVID-19 immunization resources at https://formulary.drugplan.ehealthsask.ca/COVIDImmunizationProgram and the online COVID-19 Immunization Manual available at https://www.ehealthsask.ca/services/Manuals/Pages/COVID-19.aspx Physician clinics will receive Moderna and Janssen vaccine (unless already immunizing with Pfizer vaccine for aged 12 and up). Supplies (ie. syringes) will be supplied by Public Health. Physician clinics who are already receiving COVID vaccine for administration to their patients do not need to reapply. Continue working directly with your local Public Health Office for vaccine. Record the COVID-19 vaccine administered on the line lists available at https://www.ehealthsask.ca/services/Manuals/Pages/COVID-19.aspx and submit daily. If approved, Medical Services Branch will connect your clinic with the SHA local public health office for the purpose of ordering and distributing the COVID-19 vaccine.
