Apply for a Health Card

You can apply for health cards for yourself and family online through your Saskatchewan Account. Click the apply online button below to sign in or register.

APPLY ONLINE With your Saskatchewan Account

Ways to apply for a health card:

Apply online – Click the green button above and follow the prompts to create or access your Saskatchewan Account and manage your health card requests.
Send by mail or courier – If you're unable to apply online, send a paper application.
Visit in person – In Regina, apply for a health card at our front desk at 2130 11th Ave., Monday through Friday, 9 a.m. to 4 p.m. (excluding holidays).

- Select the "Apply Online with your Saskatchewan Account" button (above)
- Select "Create Account" and follow the website instructions

I have a Saskatchewan Account but have not added the health card service

Sign into your Saskatchewan account
Link the Health Card under "My Services"

I have a Saskatchewan Account and have added the health card service

Sign into your Saskatchewan Account and select the health card service
In the "Apply for a Saskatchewan Health Services Card" section select "Next"
To start a new application, select "New Application"

I have submitted my online health card application and need to attach additional documents

In the "Apply for a Saskatchewan Health Services Card" section select "Next"
To add additional documents to an existing application select "View My Application Status"
Select "Add Documents"
