What is Remote Patient Monitoring?

Remote Patient Monitoring (RPM) is when you gather real-time information about your health at home, and send it to your health-care provider electronically. 

By participating in the program, a patient's knowledge of their clinical condition will improve and they will be able to better self-manage their health. This is a provider-driven service; patients cannot “sign up" for the Remote Patient Monitoring Program on their own.

  • Community Paramedicine

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Pediatric Nephrology

  • Prostate Oncology ​

 

What is needed to take part?

As a patient, you will need to have an electronic device such as a computer, tablet or smartphone and the ability to connect to the internet.  You will also need to have an email address to receive the invitation from your provider.  Once this has been determined, you will be able to accept the invitation and create your myMobile account. 

 

How does it work?

You will log in daily to complete a questionnaire and input vitals (eg: blood pressure, heart rate, temperature, etc) as part of your monitoring program.  Your answers will go directly to your monitoring clinician and they will support you as needed throughout your journey.
