Who can apply for a stillbirth certificate

You must be eligible to order a Saskatchewan stillbirth certificate or a copy of stillbirth registration. Some applicants also need to provide supporting documents.  

Parents

A parent listed on the original registration of stillbirth can apply. 

Siblings

An adult sibling of the stillborn child can apply.

Someone applying for you

Someone else can apply for you if you give written permission. 

They must complete an Authorization form and send it with the application. 

Legal guardians or custodians

A personal or property guardian of a parent or sibling of the stillborn child can apply.
