Stillbirth certificates
Who can apply
You can apply for a stillbirth certificate or a copy of stillbirth registration if you are:
- a parent of the subject individual
- an adult sibling of the subject individual
- a legal custodian, personal guardian or property guardian
- a personal representative of an eligible person’s estate
- a person authorized by an eligible person
- acting in the place of a parent, such as:
- the Public Guardian and Trustee
- Social Services
- First Nations Child and Family Service Agencies
- named in a Saskatchewan court order or subpoena
Types of certificates
Commemorative certificate
$35A commemorative certificate includes:
- baby's name
- date of stillbirth
- place of stillbirth
- sex
- parents' names
- registration number and date
Copy of registration of stillbirth
$55This is a copy of the original legal stillbirth registration. It includes the information recorded when the stillbirth was registered.
It’s commonly used for international purposes. It can’t be used as personal identification.

the baby's name and parent(s) information
- one piece of government-issued ID, or two other pieces of ID with your signature
