Who can apply for a death certificate

You must be eligible to order a Saskatchewan death certificate or a copy of death registration. Some applicants also need to provide supporting documents.  

Spouse

You can apply if you were legally married to, or common-law with, the person at the time of death. 

Parents

A parent listed on the original birth registration can apply.

Adult child

An adult child of the person who died can apply if they're 18 or older.

Someone applying for you

Someone else can apply if they have written permission from an eligible person.

They must complete an Authorization form and send it with the application. 

Legal guardians or custodians

A legal custodian, personal guardian or property guardian can apply.

You must include one of the following:

Estates

A personal representative of an eligible person’s estate can apply. 

You must include one of the following: 

Métis or Treaty Status

You can apply if you need a death certificate to confirm Métis or Treaty Status. 

You must include one of the following: 

Joint tenant

A joint tenant is someone who owns property with another person. You can apply if you need to prove that one of the property owners has died. 

You may need to provide one of the following: 

• a land title stating “joint tenant”  
• a title search result showing both names

Public Guardian and Trustee

A court-appointed Public Guardian and Trustee can apply.

The request requires a letter including the reason for the request and enough information to identify the person who died.  This should be on agency letterhead and signed by a person with the authority to make the request. 

Transporting the body

A person responsible for transporting the body outside Saskatchewan for final arrangements can apply.
