Who can apply for a birth certificate
You must be eligible to order a Saskatchewan birth certificate or a copy of birth registration. Some applicants also need to provide supporting documents.
Apply for your own certificate
You can apply for your own birth certificate if you’re 15 years or older.
Parents
A parent listed on the original birth registration can apply if:
- the person named on the certificate is under 18
- the person named on the certificate is deceased and supporting documents are provided
Someone applying for you
Someone else can apply for you if you give written permission.
They must complete an Authorization form and send it with the application.
Legal guardians or custodians
A legal custodian, personal guardian or property guardian can apply if the person named on the certificate is under 18.
You must include one of the following:
- a court order
- a custody agreement
- other legal documents
Estates
A personal representative of an eligible person’s estate can apply.
You must include one of the following:
- a Will
- Letters Probate
- Letters of Administration
- a court order naming you as the representative
Métis or Treaty Status
You can apply if you need a birth certificate to confirm Métis or Treaty Status.
You must include one of the following:
- a letter from Métis Nation-Saskatchewan (MN-S) or Indigenous Services Canada (ISC)
- an application submitted by MN-S or ISC with your written consent
Public Guardian and Trustee
A court-appointed Public Guardian and Trustee can apply when acting in place of a parent.
Child and Family Services
The Ministry of Social Services or First Nations Child and Family Services can apply when acting in the place of a parent.
A signed letter on official letterhead is required.
