Adding Vaccine Information to Saskatchewan's Health Record

If you received a COVID-19 vaccination outside of Saskatchewan:​

Please fill out the Immunization Submission form and submit the completed form with an attached copy of your immunization record/documentation to: Panoramareportimms@health.gov.sk.ca in order to have your vaccination information added to Saskatchewan’s provincial immunization records. This process applies to your first COVID-19 vaccine, as well as all other doses administered outside of Saskatchewan.

Your Saskatchewan health record should be updated in 3-5 business days of providing the required information and documentation. Once your vaccination information has been added to the provincial system, it will be available to your health care provider and become visible in your MySaskHealthRecord account. You can also manually add your vaccination information into your MySaskHealthRecord profile; however, the information you enter into your account will not be added to the provincial system or electronically available to your doctor. 

If you have not already done so, please sign up for MySaskHealthRecord. With a MySaskHealthRecord account, you will have quick and easy online access your personal health information, such as laboratory test results (including COVID-19 test results), immunization history (including COVID-19 vaccinations), medical imaging reports, clinical visit history (such as a hospital admission), and prescription history.
