A copy of the original death registration. These are not recommended for use as identification purposes.

Eligiblity to Apply
  • Their spouse if legally married or common-law at time of death
  • A parent who appears on the original registration of birth as mother, father, or other parent
  • An adult child of the deceased (18 years of age or older)
  • The legal custodian or personal or property guardian of the person

Information Needed When Ordering
  • Information about the deceased, spouse of the deceased (if applicable) and mother and father of the deceased
  • One piece of government-issued photo ID or two other pieces of ID that contain your signature
  • Any other supporting documents as listed in the eligibility guidelines
