Birth Certificate Types
Long Form Birth Certificate (5" x 7")

Includes the individual's full name, date of birth, place of birth, sex, registration number and registration date, and parents' full names and their province/state and country of birth.
Best choice for children 16 years of age and under, as it is generally required by government agencies, such as Passport Service Canada, SGI, Indigenous Services Canada, and for school registration.
Short Form Birth Certificate (5" x 7")

Includes the individual's full name, date of birth, place of birth, sex, registration number and registration date only. 

Please note that these certificates may not be ideal for children 16 years of age and under, as many government agencies require the additional information contained on the long form certificate.



​Certified Copy (8.5" x 14")

A copy of the original birth registration. These are not recommended for use as identification purposes and are typically used to apply for citizenship.

Eligiblity to Apply
  • The individual, if 15 years of age or older
  • If individual is less than 18 years old, a parent who appears on the original registration of birth as mother, father or other parent
  • A person authorized by an eligible individual in writing
  • The legal custodian or personal or property guardian of the individual

Information Needed When Ordering
  • Your full name, mailing address and phone number
  • Person named on the birth certificate information:
    • given names, sex, date of birth, place of birth, any previous legal names
    • parental details (names, dates of birth, places of birth)
  • One piece of government-issued photo ID or two other pieces of ID that contain your signature
  • Any other supporting documents as listed in the eligibility guidelines
