Who is entitled to apply for a Saskatchewan Birth Certificate?

  • If the birth took place in the province of Saskatchewan.
  • If the application is for your own birth certificate and you are at least 15 years old.
  • If the application is for your child’s birth certificate, they are under 18 years old and you are named as a parent on your child’s original birth registration.
  • If you are the legal guardian of a child, they are under 18 years old and you can provide court documents proving that you have custody of the child.

Important information
  • Birth certificates can only be issued for births that occurred in Saskatchewan.
  • The event must be registered with the province.
  • Birth certificates are issued using the information from the original Statement of Live Birth, completed by the parent(s) at the time of birth.
  • You are allowed to order 3 certificates per event type per person.

What is the difference between a short form birth certificate and a long form birth certificate?
A Short Form is a certified extract from registration of birth and contains the following information:
  • Last Name at Birth

  • Date of Registration
  • Registration Number
  • Date Issued

A Long Form is a certified extract from registration of birth and contains the following information:
