You need to contact us when you want to legally change your given name(s) and/or your surname for any reason other than a marriage, divorce, annulment or the death of your spouse.

Also, you need to contact us when you want to legally change the name(s) of your child(ren) or legal
 dependent(s).

If you have a common-law spouse and you want to change your name, you must file a declaration to that effect with Vital Statistics. Please contact our Customer Support Team at 1-800-667-7551 for more information.

When do I not have to contact Vital Statistics at eHealth Saskatchewan?

You do not need to contact us to request a legal name change when you:

  • get legally married (if you were born in Canada*)   
  • get a divorce   
  • get a legal annulment of marriage, or   
  • are widowed    

*If you were not born in Canada, you will need to apply for a legal name change to change your landed residency card or citizenship card.

How do I apply for a legal name change?

To apply for a legal name change for yourself when it does not involve marriage, divorce, annulment or the death of your spouse or to apply for a legal name change for your child(ren) or legal dependent(s), call our Customer Support Team at 1-800-667-7551.

A support representative will talk with you about the process and send you the application form.

You can also come in and see us at our Regina Customer Service location to speak with a support representative and obtain the application form in-person at 2130 11th Avenue, Regina.
